Objectives: The aim of this paper is to show an alternative way for an implant prosthetic planning with tilted implants which allows to obtain a good functional and aesthetic work as well. Tilted implants have changed implant prosthetic planning in the last ten years. The placement of off-axis implants, regarded as a mistake in the past, nowadays allows to place implant when a peri-implant surgery is a high risk surgery or is not applicable. Materials and methods: The authors revised the international literature about tilted implants, by manual and electronic way, in the last ten years. Articles selected and included in this study were clinical and experimental papers, where most of the implants placed were tilted and the success and survival rate were studied in relation to the placement of off-axis implants and the prosthetic load. Results and conclusion: International literature suggests that tilted implants could be a good alternative to avoid an advanced surgery or some surgery risks. The high reliability of tilted implants was shown in most of the experimental studies and, moreover, the excellent clinical results would allow for the introduction of tilted implants in clinical practice.
