This review is a picture of the various surgical interventions utilized in the treatment of post-traumatic osteoarthritis (PTOA) of the ankle joint in the highly active individual. Although nonsurgical interventions are preferred initially in the treatment of PTOA, disease severity and patient goals may necessitate surgical interventions. Arthroscopy with or without biologics, osteochondral allograft transplantation, joint distraction arthroplasty, and supra-malleolar osteotomy may all be indicated in patients with mild-to-moderation ankle PTOA. Ankle arthrodesis and total ankle arthroplasty both result in good functional outcomes in patients who have more severe PTOA.
