Madelung's or Launois-Bensaude's disease, also known as benign symmetric lipomatosis, is a rare clinical condition characterized by multiple and nonencapsulated adipose tissue deposits localized specially in the cervical region and the upper back (Type I). Accumulation of fat has also been described in the abdominal wall, lower limbs and lumbar region (Type II). This disease normally affects men aged 35-50 years and there is a strong association with alcohol abuse. The etiology is unknown, but it is frequently associated with several diseases: diabetes mellitus, hyperlipidemia, liver disease, hypothyroidism and polyneuropathy. Over the years the fat deposits achieve a large size infiltrating the nearby structures. This causes aesthetic and functional problems. There is no report of spontaneous regression. The only effective therapy is surgical excision of the mass. Liposuction can be performed in patients with masses of limited size and when there are anesthesiological problems. The disease may relapse. The authors describe a case of benign symmetric lipomatosis with cervical localization. The etiopathogenesis and treatment of this disease are discussed.
